The web-based platform connects professional carers, everyday service providers, volunteers, older people and their families, thereby creating a central point of contact for information and support.<\/p>\n<p>The aim of the project was to maintain the quality of life of older people and enable them to live independently in their own homes for as long as possible. By bringing together regional services on a single platform, the effort involved in searching for services and barriers to information are reduced.<\/p>\n<p>The underlying social franchise model promotes collaboration between professional service providers, voluntary organisations and family members.
